Home Nursing Care Services
Delivering Professional Care & Supervision with In-Home Nursing Care
In order to meet the needs of clients with significant medical conditions, Pacific Heath care givers offers nursing services. Performed by Registered Nurses, these services often include advanced care coordination, initial evaluation, and creation and oversight of short-term and long-term care plans for our clients. Our nursing staff also provides expert supervision and training to the rest of our caregivers, including Nursing Assistants, personal care aides, home care givers, and other individuals involved in the care of our clients.
Under the supervision of a nurse, our caregivers can also provide vital symptom monitoring. Because of their professional skills and training, our nurses can make healthcare referrals or recommendations to maintain or improve the well-being of our clients. This may include appointments with physical or occupational therapists, or use of durable medical equipment or oxygen supplementation. Nurses may also be able to re-order and pick up medications, set up a medication routines, and track adherence to the recommended schedule.
Cerebral Palsy Support and Care
Cerebral Palsy is a condition that affects one’s movement and coordination, caused by a problem with the brain that occurs before, during or soon after birth. We are here to help care givers of these patients with:

- Offer hope and an optimistic perspective
- Let families know that their struggles are not uncommon

Palliative Care
We offer specialized medical care to people living with a serious illnesses. This is focused on providing relief from the symptoms and stress of the illness. The goal is to improve quality of life for both the patient and the family.
Dementia Care
To support both people with dementia and their carers, adequate post-diagnostic support needs to be in place. We have come to realise that not everyone receives the same level of support, for varying reasons and we know we can make a difference
Respite Care
We recognize that caregiving, particularly for an elderly, ill, or disabled family member is a demanding job and no one is equipped to do it alone. Pacific care provides respite care services to primary caregivers as we deem such breaks vital.
End of Life Care
The final stages of a terminal illness can be a highly challenging time, depending on the nature of the illness and your loved one’s circumstances, this final stage period may last from a matter of weeks or months to several years. During this time, we come in to help to control pain and other symptoms.
Spinal Injury
This is a life changing event, it effects the entire family and can be emotionally difficult for all to deal with. With the right support and positive team attitude, however, you can achieve amazing things. Our staff support you to enjoy extreme sports, gym, athletics, holidays and ongoing education.
Peg Feeding
We take care of patients who are unable to eat through the normal food path owing to failure to chew and swallow, or having blockages in their digestive system, by placing PEG tube feeding (also called percutaneous endoscopic gastrostomy) that takes food directly into the stomach.
Physical Disability Care
We care for persons of all physical disability whether it is temporary or permanent. Home care services for a person with a physical disability are directed at helping the person meet their physical, emotional, and social needs. A person may have trouble performing activities of daily living independently. They may be unable to perform tasks such as bathing, dressing, toileting, or feeding themselves.
Acquired Brain Injury
We support our service users who have a Brain Injury (either Acquired or Traumatic) with a team of staff which meets your individual needs. We are experienced in dealing with the challenging behaviour which is often a part of a Brain Injury, through working closely and developing relationships with you and your family these can be well managed by a team that knows and understands you as an individual.
Reablement
At Pacific Health Care we offer short-term care at home, to aid our clients recovery after discharge from hospital until they are able to get back to their feet. Our usual aim becomes to enable our clients that still stand a chance to recover to regain or retain selfcare function for themselves, rather than providing input that replaces that function. We get to teach our recovering client how to cook for themselves again, rather than providing meals on wheels. This function may involve help with activities of daily living. For example, if someone has had a fall needing hospital admission, they may not be physically capable of coping with daily tasks such as washing, dressing and domestic tasks. It may also involve physiotherapy, occupational therapy, nursing care
